Outpatient Diabetes Care

Newly diagnosed patients with diabetes are seen within 1 month of hospital discharge in a multi-disciplinary clinic including a diabetes nurse practitioner, nurse educator, dietitian, and social worker. Patients transferring care from another diabetes center are seen in a similar multi-disciplinary clinic staffed by an attending physician. We encourage all patients with diabetes to be seen every 3 months.

The diabetes program is recognized by the American Diabetes Association for quality diabetes education and support. Ongoing diabetes self-management education is provided by diabetes nurse educators during all clinic visits. Patients are seen by our dietitian on average once a year during a routine clinic visit.

Patients have access to a structured insulin pump program . Many patients also incorporate continuous glucose monitors into their care.
