Be prepared to complete the following information online:

• Federal W-4 form
• MO W-4 form
• Required proof of identity – bring unexpired form(s) of identification from List A or List B and C (See list of Acceptable forms of ID)
• Direct deposit info – You will need your bank routing number and checking account number
• Emergency contact person name/phone number
• Benefit elections – If you choose to add any dependent(s), you will need provide their full name, DOB and SSN
